Is it possible for a ball bearing to come out of the carriage inside of the wheel bearing assembly?

Yes if there's enough wear/damage the balls can get loose.

I have seen the metal cage that holds the rollers fall apart too, all you need is excessive play or moisture to cause rust and it will start wearing till it spits its guts out.
