Fan belt question

Hey guys, I've got a problem on my T200 with the tensioner pulley for the cooling fan. I remember reading about it on here before but I can't find the info I'm looking for with the search function. My fan belt tensioner is not lined up with the crank pully or the fan pully so the belt is wearing out one side on the tensioner pully. What is the fix for this? Thanks.

I have seen the pivot seize so they did not auto tension, but it should not be out of alignment. Maybe a bad bushing at the pivot or a bend in the arm or mount? Should not be too tough of fix.
